At RiverFalls Home Health, we understand the regulatory requirements of making a home health care referral, and we are here to guide you through the process with ease. Our referral system is fast and convenient — we can accept referrals electronically, by fax, e-fax, or phone. If needed, a member of our team can also visit you to collect the necessary information and schedule an introduction to the services required, whether prior to a procedure or following discharge from a facility.

To help us establish a high-quality, comprehensive plan of care, we will request the following information at the time of referral:

• Patient demographics

• Insurance name and policy number

• Physician’s order for home health

• Current medication list

• Pertinent clinical documents (such as history and physical)
